CTG’s initial assessments revealed four key focus areas to support the transformation and achieve the desired outcomes: Operations, IT and Reporting, Population Health Management, and Quality. CTG and Akron worked together to implement effective population health management and team-based care best practices in each area, as well as make a cultural shift. As a result, all sites achieved PCMH recognition and met the requirement of Ohio’s program, providing Akron with regular revenue stream. The PCMH team-centered approach has improved everything from scheduling to patient outreach to coordinated care. Custom Epic integration provided a wealth of reporting and metrics capabilities, including a system of proactive outreach to parents and other providers to ensure children are getting the care they need. Akron’s new patient-centered, data-driven model is a solid foundation as they continue to transition to value-based care, which will benefit providers, patients, and payers alike.
